Transitions
Transitions – a community service program provided in the comfort of your home – can help make life a little easier for everyone in the family. The Transitions program provides a network of support for those not physically or psychologically ready for hospice. Transitions clients may still be receiving active treatment for their illness, and benefit from the support and general help of a volunteer. The Transitions coordinator helps provide ongoing volunteer support by phone or visits, identifies the needs of individuals and families and connects them with available community resources.
Transitions is a separate program from Samaritan's hospice program, and is supported in full by grants and community fundraising. Thanks to this generous community support, Samaritan offers Transitions as a community service at no cost to you. Transitions is offered to all patients receiving Samaritan Palliative Partners care.
Getting Started
Anyone, including the client, can refer an individual to Transitions. Individuals may be referred during and in the immediate months following curative treatments (chemotherapy, radiation, dialysis, physical therapy) or during palliative (comfort) treatments. Transitions services are free of charge.
